Preparing for your first visit
If you or a family member have never been treated at one of our clinics, you can view and print the registration packets to bring with you on the day of your appointment. We ask that you do not sign or date the forms until you are at one of our locations. The registration packet contains all of the documents that are needed to become a patient of Health First.
Items you will need at your visit:
- Driver’s License/Photo ID
- Insurance Card
- List of medications
- Co-pay
