If you or a family member are new to Health First Community Health Center, you can save time by completing your new patient paperwork before your appointment. The New Patient Registration Packet includes all the forms needed to establish care at any Health First Community Health Center location. Simply click the link and complete the forms online.

Frequently Asked Questions
Can I complete my paperwork at the clinic?
Yes. Please arrive 15–30 minutes before your scheduled appointment to allow enough time to complete your forms.
Do I need to complete forms for each family member?
Yes. Each new patient must have a completed registration packet.
What if I don’t have insurance?
Health First welcomes patients regardless of insurance status. You may qualify for our Sliding Fee Discount Program based on household size and income.
